Appointment policy |
An appointment is considered a mutual commitment between you and your clinician, and is subject to personal accountability and responsibility in keeping and managing the appointment. A 48 hour notice is required to reschedule or cancel your appointment, and to avoid automatic billing for payment of your session. Appointments for which you arrive late will still end at the appointed time. We do not overbook or double book, so the session is yours and is also your responsibility for managing. Please be aware that “30-minute” sessions last approximately 25 minutes, “one hour” sessions last approximately 50-55 minutes, and “one and a half hour” sessions last approximately one hour and 20-25 minutes, to allow for time to documentation in your chart. As a courtesy, you may receive a reminder phone call for your appointment; however, responsibility for keeping your appointment is ultimately yours. |
Payment for Services |
Family Focus, LLC is a private pay clinic, and does not bill or contract with insurance companies directly. As such, payment is due in full at the time services are rendered. We provide a detailed receipt to you for each visit in order that you may seek reimbursement from your insurance company (if applicable) in accordance with your benefit plan. Please keep your receipts, as we are not able to reproduce them in the future. The credit card used to make an initial appointment with Family Focus will be kept confidentially on file to be automatically charged for the full session fee in the event of a late cancellation (less than 48 hours), a no show/missed appointment, psychological scoring and write-ups, record production, or other balances that may incur. The card will be used for all family members who are seen at Family Focus. The card will need to be updated when it expires or when use of card changes to another.
For children adopted or in the adoptions unit within the OCS system – please note that OCS does not pay for missed visits and you will be responsible for the cost of a no show/late cancellation.
No direct payments from attorneys will be accepted. |
Confidentiality |
The clinic operates in a “multi-disciplinary” way, meaning that the clinicians function as a team of sorts. Staffings are held on a regular basis to review questions and concerns that we may have about a particular situation, and clinicians share the patient chart at times. Therefore, it is important to understand that the information in the chart is accessible to other clinicians in the office. However, no information about you or your care will be released to anyone outside the office without your consent or a subpoena from a judge. The only exceptions include suicide or homicide issues, or child/elder abuse or neglect. You may ask our receptionist for release forms to complete and sign if you would like for us to obtain or release records or information with other people/treaters. Children (under 17 years of age) have the right to confidential exchanges with clinicians. However, if there are issues that pose grave or immediate danger, these issues may be discussed with parents. Due to the charting nature of the clinic and the clinical focus of our work with families (not legal), custody issues will not be addressed. Please do not ask for copies of our records for these procedures, as they will not be provided. Additionally, no court ordered evaluations will be performed. |
Prescription Refills |
Prescription refills should be done at your office visit or during office hours. No after hour refills will be provided. It is your responsibility to monitor the amount of medication and arrange for refills with the pharmacy if the medication runs low. Please call your pharmacy for all non-stimulant prescription refills and have them fax the request to us at 231-7160.
Please provide at least 48 hours notice of refills needed in order that your clinician may appropriately meet your needs.
Controlled substances (including stimulant medications such as Concerta, Adderall, etc.) will be issued by your clinician on a month-to-month basis. It is your responsibility to contact the office with sufficient time to allow your clinician 48 hours to address your refill issue. There is no need to check with the receptionist about whether your prescription has been filled. It will be ready 48 hours after your request is received. Because these medications are highly regulated, prescriptions must be picked up, and cannot be called in. Also, in the event that a prescription for a controlled substance, or the medication itself, is lost it will not be re-written.

Contact and Treatment Issues |
Please email or fax your clinician with clinical concerns. Blank “Clinical Contact” forms are available and can be sent to you or used directly on our Clinician Contact Form . Please use these forms to standardize your contact. Please allow 48 hours for a reply, as your clinician may not be in on the day your request is made, or your clinician may be with patients all day and have difficulty responding immediately. Calling or contacting the office repeatedly will not facilitate this process. Please do not wait until you or your child is in crisis to contact our office. We are able to address routine concerns much more effectively than crisis concerns. If your concern involves a safety issue, please notify the front desk so that your clinician can be paged. If you have after hour concerns you may leave a message on our voicemail. If you need is emergent due to safety issues after hours please call the answering service at 379-2130.
